We ask that you bring the following on your first visit.
-Completed forms for your patient record.
-Dentist referral slip
-Recent dental x-rays, if any
-Dental insurance card & coverage information (if applicable)

The discussion of fees and insurance benefits can be one of the more difficult aspects of receiving and providing health care. We realize that every person’s financial situation is different. Our purpose is to help you achieve a healthy, confident smile while respecting your budget. For this reason, we have established fair and reasonable fees for our services and offer payment options that will help you receive the care that is necessary.
Our office accepts cash, personal checks, money orders and credit cards (MasterCard, Visa, and Discover). Credit arrangements can be made through CareCredit which offers no application fees, and no or reduced interest payment plans. If you choose to use CareCredit, please establish an account prior to receiving care.

As a courtesy to you, our office will submit a claim to your dental insurance company for any treatment provided. Please remember that your insurance plan is a contract between you, your employer or retirement plan and the insurance carrier. We are not a party to this contract and have no control over the details of the contract. However,

We are currently in-network with several insurance plans including Delta Dental, MetLife PDP, and Superior Dental Care. While we accept other insurance plans, it is best if you contact your individual carrier to be aware of your out-of-network benefits.
We are not providers for Medicaid, Medicare, or Workers’ Compensation.

Our Response to COVID-19
- We follow or exceed recommendations and guidelines made by the American Dental Association (ADA), the Centers for Disease Control and Prevention (CDC) and the Occupational Safety and Health Administration (OSHA).
- We will continue to schedule appointments in a manner that promotes social distancing and allows for adequate time for us to sanitize all areas of use in between appointments.
- Face coverings are not required, but may be worn based upon personal preference and/or risk level.
- Dr. Beckner and his team will continue to follow current guidelines for all recommended personal protective equipment (PPE).
